COVID-19 Health Screening Form

Please assist us by confirming your appointment within 24 hours receiving your email or text appointment reminder.   We appreciate your understanding of our confirmation policy as it allows us to see our patients in the safest environment possible & minimize wait times.

All screening forms must be completed and submitted at least 1 hour prior to being seen.

 Mask mandate still remains in effect in a healthcare setting - “CDC’s recent mask guidance for fully vaccinated people does not apply to healthcare settings; face coverings & social distancing are required while in the dental office”

Please carefully review our updated check-in protocol:

  1. Health Screening Form: If it is not filled out by the time of your appointment, we'll ask you to fill it out prior to being admitted into the office.

  2. We ask that adult patients attend appointments alone if possible, and minors attend with one responsible party only to limit the number of people in the waiting room. Our coffee and tea stations are also temporarily closed.

  3. Please text us at (925) 272-7868 once you are parked. Once your chair is ready we will text back to notify you to come into the office. If you are early or late please first call us at (925) 272-7868 to let us know.

  4. Please arrive with a mask on. If a mask is not worn by a parent or guardian we will be asking that you please wait in the car for the duration of the appointment.  **Mask mandate still remains in effect in a healthcare setting**

  5. Please use the hand sanitizer at the check-in counter when you arrive. ***Please remember to brush & floss your teeth at home prior to coming for your visit as our toothbrushing station will be closed***

  6. After your visit: If you are able to schedule your next visit, please stop by the front desk to do so. If you are a minor and your parent is not present to schedule, please expect a text from our office that same day to schedule your next visit.
