1. Submit Your New Patient Forms
Once you’ve submitted your Virtual Consultation request above, please click the link below to submit your New Patient Forms online.
2. Submitting Your Photos
Please take a few photos (as described in the link below) and email them to admin@tri-ortho.com before your consultation. While we will eventually need to take full orthodontic records at your in-person visit, these initial records will help our doctors come up with a tentative treatment plan prior to your virtual visit.
3. Preparing for Your Virtual Consultation
We use a secure HIPAA-compliant telemedicine platform Doxy.me for our complimentary virtual consultations.
We’ll send you your personal meeting link via email once you’ve scheduled your virtual consult. Simply click this link to start your virtual meeting on your computer (recommended) or on your smart phone (the mobile version of doxy.me will not work on Iphone 6 or older).
